Scholarship Application

Upon submission of your application, you will receive a confirmation email from Human Resources. If you do not receive a confirmation email within 48 hours, please email eaglepicherscholarships@eaglepicher.com as there may be a problem with your application.

Please enter name and complete address. Change of school requires written notification to the address on this form prior to checks being issued.
All scholarship recipients must maintain 3.0 combined GPA for high school seniors and 3.0 cumulative GPA for college students to qualify. Grades must be submitted for each semester in order to receive funds ongoing.
